Before You Ship

When planning to collect your specimen, Diagnos-Techs™ recommends you first plan the date you will ship the test kit back to Diagnos-Techs, and then collect the final specimen the same day. This ensures maximum specimen viability for our lab. If you are collecting for a panel that requires only one day of specimens, your final specimen should be collected either the evening before or morning of the day you ship. If you have additional questions about collecting a specimen please check here for detailed instructions, or check with your healthcare provider.

  1. Write your name, address, gender, birth date and collection date on requisition form.
  2. Write your name and date of birth on all vials.
  3. Be sure required test orders are marked on the form. If not, please contact your provider for test orders.
  4. Include payment check or credit card information and copy of Medicare or insurance card if applicable.

Instructions For Patients Residing in the United States

Free Shipping for a Single Kit in the United States!*

Diagnos-Techs test kits contain a pre-addressed return label and include free shipping which guarantees a maximum of three business days from the date you ship on kits shipped within the U.S.
Just place your specimens in the box, seal it shut and drop off at your most convenient UPS location.

The return label included in your test kit guarantees a maximum of three days from the date you ship until it is received by Diagnos-Techs. If you prefer a faster level of service you may click here to explore your options. Please be aware, you will be responsible for all charges related to a selection of faster service. If you have lost your prepaid label please click here to create another one. In the case of label replacement, you will not be charged for this service.

It is recommended that kits be returned in advance of or after a public holiday. For a list of US public holidays and closures, please click here .

  1. Place vials (sealed inside the ziplock bag included with your kit), requisition (test) form and payment into kit box.
  2. Tuck front flap into box. Seal with UPS mailing label. Place label within dashed lines and adhere over front edge of box.
    IMPORTANT: Ensure that the portion of the label containing the bar code is not bent around any corners on the box.
  3. Please send from your most convenient UPS location, or schedule a pickup through UPS by calling UPS at 1-800-PICK-UPS (742-5877). Additional charges may apply to schedule a pickup.



Instructions For Patients Residing Outside of the United States

It is the responsibility of health care providers and patients to ship completed test kit boxes back to Diagnos-Techs Laboratory within the time frame indicated on the requisition form. Diagnos-Techs is pleased to offer its international clients a discounted shipping rate on their return shipping through UPS. To benefit from this service, please click here and enter your information.

All shipping materials necessary for UPS shipment have been included in your kit. The following instructions explain how to return your test kit to Diagnos-Techs through UPS.

  1. Go to UPS returns , choose the service you desire, pay, then print out the shipping label, customs invoice and your receipt.
  2. Place vials (sealed inside the ziplock bag included with your kit), requisition form and payment into kit box. (The requisition form is also the test form.)
  3. Tuck front flap into box. Seal with packing tape.
  4. Insert the kit box into the opaque UPS Express Pak mailing pouch that was included with your test kit. If you did not receive an Express Pak pouch with your test kit, please contact your provider or obtain one from a convenient UPS location.
  5. Sign your shipping label and all three copies of the commercial invoice. Place the three signed copies of the commercial invoice and your shipping label into the "International Shipping Documentation" ziplock pouch with the shipping label and barcode visible. Seal the pouch.
  6. Peel off the backing of the re-sealable, clear plastic "International Shipping Documentation Pouch" and affix it to the UPS Express Pak.
  7. Please send from your most convenient UPS location, or schedule a pickup through UPS here or by calling UPS in your area. Additional charges may apply to schedule a pickup.

If you choose not to use UPS, delivery charges still apply. International deliveries should be addressed to our physical address only, listed below. Do not address to the PO Box. Deliveries can be made Monday through Friday via a private courier of your choice. When shipping to Diagnos-Techs from outside of the United States, please be sure to fill out and include a customs form.
